- Social Security Card
- Income: (Must provide for ALL members in the household)
- Check Stubs (For the past 30 days)
- Social Security Award Letters (For the current year)
- Unemployment Benefits
- Child Support (Payment history for the past 30 days)
- TANF Letter
- Annuity Award Letter
- Adoption Subsidy
- Rent Change Notice (If enrolled in housing program)
